The coverage is restricted to employees working in the hospitalization sector and adherents to the national social security fund and their dependents.
1. Any hospital desirous of becoming an adherent to the national mutual fund of health in Lebanon should fulfill the following conditions:
a- Subscribe all its employees having social security coverage unconditionally (Note that adherence is compulsory for all dependents: wife and children) .
b- All persons interested in becoming adherents should subscribe within a two months period from the date of notification by the fund on the initiation of adherence; they should also provide the fund with the names of employees and their family members. (Benefit is immediate).
c- At the end of this period, special conditions become effective as far as the adherent is concerned and the fund’s board of directors shall then decide whether to accept or reject his adherence. Excluded is any new employee who adheres to the fund subsequent to his adherence to the social security within two months from the date of registration (benefit is immediate).
d- It is preferable to subscribe the employee’s parents – who are dependent on him and registered in the national social security fund- to the national mutual fund of health during the adherence subscription term of the adherent. Otherwise, if the adherent wants to subscribe his parents who are dependent on him to the social security fund, special conditions become effective as far as the parents are concerned, and the fund’s board of directors shall then decide whether to accept or reject his demand.
2. The mutual fund gives a term of two months for the adherence of each person who newly become an adherent’s dependent (a newborn, the newly-wed partner and the two parents after getting social investigation). Should this term pass without receiving the adherence, special conditions become effective as far as the adherent is concerned and the fund’s board of directors shall then decide whether to accept or reject his adherence.
3. Special conditions:
a- Filling out medical questionnaire by applicant.
b- Enforcing a waiting period on the applicant as determined by the fund’s board of directors.
c- Enforcing a one-year waiting period on the applicant from the date of acceptance of subscription in case of birth, slipped disk, chronic diseases (heart, dialyze, …).
d- Enforcing a fifteen-day waiting period in case of accidental emergencies.
e- The submission of employee’s parents to a medical checkup at a physician to be named by the mutual fund.
4. In case of cancellation for any reason on the part of any adherent, he is required to pay the value of the full annual cotisation notwithstanding his adherence period.
Should he decide to renew his adherence, related hospital’s administration should provide the fund’s administration with an application clarifying causes for cancellation and renewal of former adherence so that it decides accordingly.
5. Documents that the hospital should submit to the fund’s administration:
a- A copy of the birth certificate for the subscription of a newborn.
b- A copy of the marriage certificate for the registration of the wife.
c- A copy of the social security investigation for the registration of the parents.
d- A copy of the "employment application" sent by the hospital to the national social security fund as to the employee who transfers his social security from a former corporation to the hospital.
e- A copy of the "employment application" sent by the hospital to the national social security fund and a copy of the national security card regarding the newly-covered by NSSF employee.
